US Futsal Registration.

Print a copy of your Certificate of Intent DO NOT SEND A CHECK.

- Complete USFF Coaches Paper Form.
- Write a check for $150.00 deposit per team (payable
to Spokane Futsal)
- Send completed Team Application,
Coaches Certificate of Intent or Registration and deposit check to
Spokane FUTSAL

Once your team has been accepted have each of your
players register with US Futsal using one of the following methods.
Spokane Futsal will match player registrations with the team
application. Coaches will be notified if any player listed on the
team application does not register.

In order to participate in our league or
programs, you must be a current member of the US Futsal Federation.
The following forms are needed for your annual membership, player card and
insurance. Membership is valid from Sept 1 - Aug 31st.
Youth players must have age verified prior to first game. The
following are accepted: WSYSA/USYSA player cards, State issued birth
certificate (hospital issued are not accepted), SYSAS or SpVJSA signed
official team roster, Washington State drivers license/ID.
